Yes, you're right. Maybe the module can be improved by checking
the call table after rule initialization to record active calls that fall under
the given rule. Right now, a new rule keeps track of new calls only.

What I have seen is, when a rule is added, it does not, for some reason,
apply to calls already running that falls under that rule.  Only new calls
are processed.  In my case, I found the new rule to be working alright the
next day, without any changes, and any reboot of GnuGK.  So, I guess,
CapacityControl starts tracking calls once a rule becomes available.
