See description of NetworkInterfaces at
http://www.gnugk.org/gnugk-manual-4.html
Replace 192.168.1.1 with the GK private IP, and 10.0.0.1 with your real IP.

You will need to forward the proper ports to the GK.
Typically, these will be UDP 1719 (RAS), TCP 1720 or 1721 (signaling),
TCP ports for H.245 (set H245PortRange to match),
and UDP ports for RTP (set RTPPortRange to match).

See manual for other port ranges if you are doing something exotic.
Or, put the GK in the DMZ. If you do that, make sure the status port
and any other admin interfaces are secured.
