GNUGK behind NAT

I need help to setup GNUGK behind NAT.
EPs(real IP) <-------> | Real IP NAT Server | <-------> GNUGK

I have put a small list of NAT traversal strategies and whether they are supported and require media proxying by GNUGK if on the outside of the NAT.

Calls from behind NAT.
Registered                 supported             proxy.
unregistered not supported. Note: Patch pending to add support (proxy).

Calls to NATed EPs
Registered not supported returns reason "User unreachable"
GnuGK aware EP       supported           proxy  (using GNUGK Nat feature)
GnuGK on NAT supported proxy (only to GNUGK outside NAT)

NAT Traversal Strategies (require help from NAT box or external helper)
Port forward               supported    proxy.
Port forward Masq. supported No proxy. (using the external IP of the NAT) STUN supported No proxy (not work with some NATs) UPnP supported No proxy (Require compatible NAT box)

Calls between EP's behind same NAT
Registered                 supported          No proxy

Note: You will need to enable support for NATed EPs in the settings.

I've tried with routed mode gnugk -r -tt but when i make call gatekeeper is trying to open rtp proxy channel
