Dear Hernan,
Your scenario is not really difficult. First of all, you can register
your DMZ-ed gw endpoint by manually put the record in your ini file. So
GNUGK will always have your gw's public ip rather than masq'ed IP. If you go
to the status port to take a look of those client, you might probably found
that the gw with masq'ed IP.
If you see a public IP and it still not working, try to map ports TCP
1710~1725, UDP 40000-59999 (according to your ini setting to those potr) to
your GW. I think it should work as my IP-PHONE behind NAT works in this way.
And also netmeeting is not a good client software at all. You can try
openphone from openh323.org or sjphone from sjlabs.com
