One other observation I have just made is:

With Faststart on H.323 EP,
Ophone dials H.323 EP, I have audio on both ends
H.323 EP dials Ophone, there is no audio on both ends

With normalstart on H.323 EP, I have audio on both scenarios.

In that case I'd check if your H.323 endpoint is trying 
to execute TCS/MSD procedures before the CONNECT 
message is sent. If not, try to enable this behaviour 
(early media / faststart/ etc).

We had similar problems with our Cisco.
