You should provide more details like:
1) what kind of NAT you are running, what ports are open;
2) where the particular endpoints and gatekeeper reside
(public IP, private IP, inside NAT box);
Be aware that ATA-186 does not like to cross NATs,
so your gatekeeper should be located inside NAT box
(with access to both network interfaces - private and public):

You should also enable Proxy=1 in gnugk config file.
