That's not crap that's reality. You can publish discuss get reviewed approved as much as you want that does not remove all responsibility if flaws are found in the approved plan once implemented. An approval and review procedure never changed the fact that ultimately the person or team doing a proposal had the most visibility and brainshare to spend on it. If it works out they get the fame. If not they get the shame. Bad karma can be earned back. Given the state our old bug triaging was the bug zapping team will have lots of opportunity to win good karma. -- Nicolas Mailhot
