I think you've misunderstood the diagram, which is Ville Skyttä's attempt at explaining the "fedora.us update submission process" in a non-textual way: http://www.fedora.us/wiki/PackageSubmissionQAPolicy#updates It simplifies the package update process for the trusted package maintainers a lot, since they no longer need to wait for reviews and approvals from other people. The diagram does not cover submissions of new packages.
