The mgr_initial_modules setting is only applied the first time your
cluster starts (it's useful if you're pushing out a static
configuration from another tool).  Subsequently you must use "ceph mgr
module enable".

The dashboard docs probably shouldn't be mentioning this setting, as
it applies to all modules, but is only useful in the specific case of
rolling out a configuration before the cluster has started for the
first time: https://github.com/ceph/ceph/pull/22808

BTW, you're looking at the 13.x documentation, the 12.x documentation
is at http://docs.ceph.com/docs/luminous/
