    Release Notes: This release does not include updates for Vaccine Information Statement (VIS) codes and maps. Release notes and files for VIS code updates are distributed in separate release cycles.


    General Note regarding this release: Codes with “Last Update” dates after September 13, 2019 reflect the changes and additions in this release and include the following code set updates:


    1. CPT / CVX Code Updates:
    • New CPT Code Added: Code 90694 “Influenza virus vaccine, quadrivalent (aIIV4), inactivated, adjuvanted, preservative free, 0.5 mL dosage, for intramuscular use”. This highly specific CPT code has been mapped to CVX Code 150 “Influenza, injectable, quadrivalent, preservative free”
    • CPT Code Description Updated: Code 90734 description has been updated by the AMA to “Meningococcal conjugate vaccine, serogroups A, C, W, Y, quadrivalent, diphtheria toxoid carrier (MenACWY-D) or CRM197 carrier (MenACWY-CRM), for intramuscular use”. No CVX mapping change has been made.
    1. NDC Code Updates:
    • There are no new NDCs in this release.
    • The following NDCs have been retired by the FDA – in some cases, these updates were previously identified as no longer active by the CDC. These NDC retirements appear to include some data clean-up of older items by the FDA from the active files.

    1. NEW - All NDC11 to NDC10 Reference. A new downloadable crosswalk file has been added to this release as a reference tool. It provides a list of all unique NDCs included in the vaccine code set files. This file will be useful when a user receives an NDC in one format but needs to easily cross-walk that NDC to a different format. This file is not yet available on the IIS Vaccine Code Sets website. It can be downloaded using the following link:https://www.cdc.gov/vaccines/programs/iis/downloads/NDC11-NDC10.txt

    Examples for using this reference table include:

    • Bar codes to CDC files. Bar codes include the NDC10 Unformatted value. Using this reference table, the user can translate the NDC10 Unformatted code to the NDC11 Formatted value which is the format provided on CDC vaccine code set files and the NDC to CVX maps.
    • DailyMed research. A user may wish to look-up an NDC from a CDC Unit of Sale or Unit of Use file to see the information about that drug in DailyMed. In this case, The CDC NDC is presented in the NDC11 Formatted value, but the search in DailyMed will require entering the NDC10 Formatted value.
