No, vcsa is essentially just a copy of what is on screen. It's not an audio device, although it can be piped out to a speech synthesizer using various methods such as what Fenrir uses to monitor vcsa to know what has changed on screen or what has been added. As far as I know, the vcsa system doesn't preclude the use of any output device to speak the content of the screen. Fenrir just hasn't yet implemented a serial or USB driver for hardware speech output. It's a limitation of the software right now, not a permanent one, and not a limitation of text mode accessibility that is exposed to userspace.
