You're probably better off using your Windows screenreader for Cygwin. Though their goal is to make Unix things compile and work correctly, reality is that the source frequently must be modified. For example, they have their own version of Screen since the standard version doesn't work even though the configure script mentions Cygwin.
So, get Cygwin working first and then worry about exotica like separate screenreaders.
