I'm beginning to suspect flakey hardware in your case. I would consider
trying the controller in standard IDE mode, maybe using Linux Softraid,
to see if anything changes.

To clean your disks the safe and slow way, download a utility from your
disk vendor and reformat them on a standard IDE port (you could also
reinitialise the boot sector). Once they're clean and tested, you can
plug them back into the raid, and use the BIOS to initialise the array.
