Yes, you include enough text to maintain context, but only what's relevant for that.

Most people prefer to read things in chronological order, with the question first, followed by the answer. Top posting reverses that. Office workers file things with the most recent paper in front, so they're used to reverse chronological order, but the rest of us don't like it. And if you read the list guidelines, you'll see that they tell you not to top post.
I've been in the situation when directing multiple questions to a specific person in an email, that person put his responses after each question in a different colour so that they stood out, which is fine.
I've also been in the situation where the mail recipient top posted the responses to the questions with each response numbered to match the order of the questions, which is also fine.
In my 46 year career, the last 18 months is the only time an organisation has used Microsoft Outlook, all the rest of the time the organisations used Domino/Lotus Notes, which I have always considered to be far superior to Exchange/Outlook. The only reason the current organisation is using Outlook is because they are moving to a number of Microsoft applications in the cloud.
