The most suspicious from the top suggests maybe the quirk isn't correct, or possibly it's not getting enough power, or just some incompatibility between the host USB controller and the one on the client. Best suggestion I can offer is ensure it's connected via an externally powered hub. The way the hub works, the command and data stream is normalized, and the device properly powered. All of my USB problems with drives have been solved by using such a hub. But it is still possible it's some other problem in which case the place to go is the usb list.
