trouble with a Intel HD Audio card

My Dell laptop has this soundcard which is giving me a few problems.
At first the microphone would not work, I found out I need to load the
driver with 'model=ref', because it has the SigmaTel STAC9200 chip.
Now I still have a couple of outstanding problems.

1) Sound from microphone not routed to headphones/speakers
Apparently this card is made to be controlled through software, so
that complicates things. My microphone now works, I can record myself,
but I don't hear the sound in my headphones, as I expect to. This
makes it less obvious to tell if the recording is working and what
volume it's set to.

2) After suspend-to-ram, headphones don't work
I've been experimenting with acpi sleep and I've noticed that after I
resume from suspend, suddenly there's no sound in the headphones, it's
played through the speakers, even though the headphones jack is
plugged in. I have to reboot to fix this (or possibly just reload the
module, I don't know).

So what to do?
