[XFree86] Alt-Key Generation of Eight-Bit Ascii

I often use the alt-key to generate eight-bit characters directly
into text editors and such.  I have recently discovered that this
ability comes and goes.  It works in XFree86 4.1.0 and 4.2.1 but
not 4.2.0.  In each case, all settings in my user-account were
identical.  My questions are:

1) Where in the documentation can I find this capability discussed?
2) Are there initializations for XFree or other software that can
   effect this?  User level?  System level?
3) Where is the alt-key -> eight-bit implemented in the source?
4) Is this capability part of a standard?

Any tips or pointers very appreciated.  Thanks!
