Some old games used really bad ways to draw cursor (mixing ddraw with gdi, redrawing entire screen). For Wine it's the worst thing possible - it has to do multiple round trips to X server and back for each draw operation. This is without going into details. There are not much you can do about this. If there is a way for the game to disable it's cursor and use standard system cursor - try it. If that does not work - you might have to wait for the Wine's own DIB engine which might (or might not) fix the problem.
