DOS VGA, add minimal SVGA support

After this patch, some applications using SVGA modes seem to work.
However, most SVGA apps use 32-bit DPMI, which does not work.
Almost all VGA modes really need to emulate reads/writes to 
display memory in order to get decent results, this is still missing
because it requires lots of work. This patch also reorganizes code
in int10.c because five nested switch and if-else blocks make
things way too complicated.


Changelog:
  Add support for moving VGA controller window in framebuffer
  using SVGA interrupts. Detect correctly color plane modes and
  linear modes, including Mode-X. Moved VESA interrupts into
  separate function and implemented few more stubs.
