It's a CMI8738 based card, Innoax Audio Extreme 5.1 with both 
coax and toslink in/out capabilities. 

alsamixer has a settings for 'Loop mode'.

The Coax and Toslink connectors are mounted on a separate slot connected to 
the soundcard via flat cable connector. There is some buffering components on 
the extra card with the connectors.
