Not a solution but I found this bug report and short discussion:

Hi Darrell,
the file association allows to change the settings for all files using the "All" group, just set it to external view.
That is what I did (or perhaps was the default) and it works like a charm.

Kind of not fully intuitive/obvious to me. I have that set but upon further inquiry there are sub levels in the 'all' category.

The top level shows 'Show file in separate viewer', but expanding the top level 'all' widget shows an 'all' and 'allfiles' option that need the same configuration.

I'll see how that goes.
