I had a similar issue, where the default mixer would always be set to the onboard sound and not my current sound device, often had to set the default device every login, I don't remember the solution as I've since switched to wmamixer but if I figure out any more I will let you know. Bluetooth devices are technically considered additional sound cards so it makes sense this would happen.

You can also use the newer kmix from Plasma if you already have that installed. Seems to work better but ofc the interface doesnt fit in quite as well.
