Yes, it sounds like an OFED specific issue, involving the OFED provided
ib_isert being complied against the distro provided iscsi_target_mod.

Really, iscsi_target_mod needs to be included in OFED, and OFED provided
ib_isert should not be depending upon iscsi_target_mod provided by the
distro kernel.

Yes.  However, v4.2 is recent enough that upstream should have all of
the hw drivers you need.

Are you even sure you need OFED to start with..?
