The patch simply allows kernel Makefile to accept PRE_CFLACS (OFED includes in our case). It probably won't apply cleanly on target-pending/for-next Makefile But can be applied manually.
Yes, ib_isert will compile against the installed OFED version.

the user guides or anywhere else? I was unable to find anything
like this.

Unfortunately it's not documented anywhere AFAICT. When using 3'rd party inbox modules (which depends on ib_core and friends)
against OFED installation, symbol resolution will break.

Other than what I suggested, I don't know of a better solution.
