Thanks Nicholas.

I've downloaded the firmware and overwritten the version I had, rebuilt the initrd image and rebooted.
Now dmesg shows:

[    1.592448] scsi0 : qla2xxx
[    1.592839] qla2xxx 0000:09:00.0: skipping scsi_scan_host() for non-initiator port
[    1.592939] qla2xxx [0000:09:00.0]-00fa:0: QLogic Fibre Channed HBA Driver: 8.03.07.12-k.
[    1.593032] qla2xxx [0000:09:00.0]-00fb:0: QLogic QLE2462 - PCI-Express Dual Channel 4Gb Fibre Channel HBA.
[    1.593140] qla2xxx [0000:09:00.0]-00fc:0: ISP2432: PCIe (2.5GT/s x4) @ 0000:09:00.0 hdma+ host#=0 fw=5.06.05 (9496).

I'm running the Debian Linux distribution and the /lib/firmware/ql2400_fw.bin file came from the firmware-qlogic package.
I've checked and there is a bug report already asking for the firmware to be updated.

	http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=659217

I'm still running on the kernel that I compiled on the 14th Feb.
I'll let you know if it has issues again.

Regards,
