Thanks, indeed.

Meanwhile I tried to reproduce the issue with my two newly added FW643-e2,
doing several cold boots, several warm boots, and a large number of
modprobe -r/ modprobe cycles, but the failure did not come up yet.

Instead, the latest modprobe -r/ modprobe loop eventually caused a
JMicron JMB381 [197b:2380] (revision 0) to fail in ohci_enable,
ohci_update_phy_reg(card, 4,...).  This controller is of course not
influenced by either your 3.10 patch or by my 3.14 patch, and it is an
notoriously fragile controller anyway.
