Random Session Expires

Hi,
My setup is
Debian Etch
Apache2
PHP5.x
Squirrelmail 1.5.1
Imapproxy
Courier-imap
userdb (authentication)
squirrelmail plugins installed
html_mail / spam_buttons / calendar / notes / squirrelspell

As our client base  has increased some of our clients are reporting random session expired message from squirrelmail when they try to login into the account even for the first time, however they are able to log in after 3-4 attempts, each time getting the same session expired message.
I am not sure why these random session expires are happening even when someone is logging in for the first time during the day.
Appreciate any help in this regards.
