ok, I contacted the qmail mailing list and they recommended I use sendmail
instead of SMTP to send the mails. Not the old sendmail software but the
sendmail option in the squirrelmail options...

Server Settings -> Sendmail or SMTP -> Sendmail

I did this and got a totally different header...

...so I am going to stick with this configuration. Apparently no one knows
the solution to my original problem (Using SMTP instead of Sendmail)
