I'm running SM 1.4.10a-1.fc6 with dovecote IMAP - I do get '!'.
The headers on that message (I don't get many with high priority set) 
received from Outlook/Exchange say:

*X-Priority:* 1
*Priority:* Urgent
*Importance:* high


Are any of the other signs showing up, 'A' and '+' etc?
