Try renaming the "password" column.
 - rename it in the DB, change the squid.conf helper parameter, then
reload squid config.


Perhapse show us what you are entering on the command line test for
username and password?

Perhapse try using the --debug option? to get the helper to record in
cache log (or on screen for the manul tests).
