After reviewing bugs 3279 and 3483 I am pretty sure we can draw a path
that will help with this issue by minimizing couple culprits.

I have seen couple snippets from your squid.conf but I want to build a
new picture.
I feel sorry for you that you lost so much data but before you jump into
the water and push the power button again on the service I would like to
make a suggestion:
- Try to run the service for 48-72 hours on ram ONLY and see if you are
having any issues.
Now a question? are you using SMP by any chance? here your squid.conf
will help to understand what is your setup.
If you can get some details on the machine using lspci, dmesg or other
tools it would help to understand the issue more.
If you do wish to at-least find the main cause to the issue don't try to
just "fix" the issue.
First start with basic tests and make sure that the service is stable to
run a period of 48-72 hours straight with no reconfiguring and\or restarts.
Once we will get there with little steps we might be able to even solve
the issue.

Understood. Those two bugs 3279 and 3483 are very frustrating. If your
experience is anything like the rest of ours now the bug goes back
into hiding.
