I compiled 3.4.3 from source. This does seem to work now, albeit slow to fall back.

I think I should have searched 'squid happy eyeballs' first. Apologies, finding more details now and reading into it.

There have been a number of small issues in this area found and fixed as
of the latest 3.4.3 release. Please try an upgrade to the latest you can.
