Hey,
The issue is not 100% related to squid.
You can try to use default squid.conf and then see if you get the same
issue.
If the issue is only related to Squid configuration we can try to help you.
While if the issue can be file permissions or any other non-squid
related issue I doubt we can help you more then just make sure that the
permissions are right or wrong for the executable file or any of it's
upper level directories.
If you want to make sure This is something related to squid try to run a
"fake_url_rewrite" program\script.
An example is here:

I am not sure how much new squidcode is compatible with old squidguard
but it should be by default.
Can you share "squid -v" output?
