Did you tried default squid? apt-get install squid3 Maybe something else uses ldap port? Try with (if I am not wrong): debug_options 82,0 84,9 Do you have wireshark? Can you capture ldap requests on windows server from Ubuntu? Do you have firewall from Windows Server on? >From my practice it is better to switch firewall off. Configure all services. Capture ports. After slowly put it up. It is very strange... I got one time working the Squid 3.1.x -> W2k3 on U12.04, but Squid 3.1.x -> w2k8 on 12.04 doesn't worked by me. Oh yeah, one more thing chmod... Do you have right privileges? Do you work from root? Based on my practice helpers are working only on proxy user or root.
