Nothing has changed in that regard AFAIK.

IMO we will probably just remove the *_size option entirely.

If I am understanding Alex right that depends on the other setting icap_preview_enable ON/OFF. Squid should be sending the negotiated preview size (ignoring the icap_preview_size config), unless it has preview disabled entirely.
