Hello,
So there is a bug.  This was fixed in some branch, but I do not know
if it's fixed in 2.7-STABLE[4,6,7].   I was seeing the same issue
where sometimes I can get the storeurl_rewrite to work, but other
times I can't. This is the bug report;

I have seen other threads address this issue but they appear to be
stale or closed ie:

Was wondering if anyone knows which branch this fix was applied to.
Also I tested with 2.HEAD so I am not sure if the fix was committed.
tried: squid-2.7.STABLE4, squid-2.7.STABLE6,
squid-2.7.STABLE-720100211,  squid-2.HEAD-20100212, and 2.7-STABLE7
