Item 2 has a patch pending, coming to a STABLE release near you. I'd
recommend running redundant squids due to #1, however.

COSS is not stable enough for production use in 3.0. The bugs fixed in
2.7 were not yet ported over. They are not expected to be done before
3.2 either, but depending on the fix maybe back-ported when done.
Patches against 3.HEAD are always welcome and Alex would like some help
if anyone is interested in seeing COSS stable anytime soon.
