Just for clarity, squidclient is a very basic HTTP query tool, and has
nothing to do with X-Fowarded-For headers. X-Forwarded-For headers

Well... The squid-dev list charter states that it's for those
"interested in developing and/or testing experimental squid code
during the development and not for general users." *shrug*
Given the information provided so far, ("squidclient -U user -W
password mgr:active_requests" only shows one entry) I'd have to
surmise that you are not querying an active Squid server. Do you see
an entry similar to...
1240946289.761 1 127.0.0.1 TCP_MISS/200 87722 GET
cache_object://localhost/active_requests - NONE/- text/plain
...in your access log?
