Just for clarity, squidclient is a very basic HTTP query tool, and has nothing to do with X-Fowarded-For headers. X-Forwarded-For headers are added by Squid (and can be used in ACLs and logs). What you would are asking about is the availability of X-Forwarded-For information in the Cache Manager menu.

Given the information provided so far, ("squidclient -U user -W password mgr:active_requests" only shows one entry) I'd have to surmise that you are not querying an active Squid server. Do you see an entry similar to...
