If an IP is included in the URL text it will be matched against.
It won't match IP if a domain is given.

both anywhere in protocol, domain, path, and query parameters.
But thats not an external acl, which you asked about...
and at "/etc/squid/domain-nakal.txt"

base concept it to listing *avatars* ( include *.avatar.* avatar.*.*
*.*/avatars etc etc ) to that acl

Depends entirely on the regex configuration.
The pattern by itself is useless to say what it matches.
