You are using Squid 3.0.8 release. Amos is recommending that you use the
code from the development tree, which is often referred to as HEAD or
trunk and which will become Squid 3.1 eventually.

Squid 3.0 does not have the follow-x-forwarded-for support you need.
Squid3 trunk has it, but I would recommend applying the patch posted by
Amos at http://www.squid-cache.org/bugs/show_bug.cgi?id=1628
Without that fix, the feature does not work in my tests.
