Squid will use /etc/hosts even if it internal DNS lookups are enabled.

Did you see the user guide section on acceleration (http://www.squid-cache.org/Doc/Users-Guide/detail/accel.html)?  Use the third option (set httpd_accel_uses_host_header on), and use a redirector.
