swap.state grows until you run "squid -k rotate" or restart Squid.

result of this is swap.state.clean about 20MB and squid hangup. still visible in processes, but refuses all requests.

Odd.. this is not expected, and not reported by anyone else.

Is there any info in cache.log?

If not it would be interesting to know what your Squid is doing. Attach to it with gdb and print a stack trace.
