Has anyone come across this before and is there a fix?
Yes. Check your cache.log, the reason why this is rejected is clearly logged (and it is broken - hint: see above).
Have a read through one of the archives listed at http://www.squid-cache.org/mailing-lists.html , the answer to this question (as well as a way to work around the brokenness) has been posted about 100 times already this week...
