[Redhat-s390-list] DASD Formatting Failure

I'm using the latest VM initrd and kernel.  When I IPL the system, and start
rhsetup, the first thing I see is the DASD initialisation dialog.  I tell it
that I want to make /dev/dasda my root, and /dev/dasdb my /usr.  Then I'm
asked if I want to use the cdl or ldl format.  I select ldl and hit "enter",
which is immediately followed by:
          Memory fault
                       formatmnt
                                Stopped installation. No cleanup is done!
                                                                         #

On the VM console, I get this:
