Deadlocked database?

I seem to have a problem with a locked database, and can't seem to find a way to recover. First, the version info:
EdgeServer 3 rev 2 (partially upgraded to rev 3)
db4-4.1.25-8
rpm-4.2.2-0.14

All rpm-related commands (rpmq, up2date, rhn_check, etc) lock up on a futex() call, which appears to be db4's locking mechanism. I assumed this would be pretty easy to fix.. copied over db_deadlock from a machine with db4-utils-2.1.25 on it, ran that.. and low and behold I get this:

db_deadlock: Program version 4.1.25 doesn't match environment version 4.1.24
db_deadlock: open: Invalid argument

The machine was not recently upgraded from db4 4.1.24 -- it's been running 4.1.25 from day 1, as far as I know. db_recover, and surprisingly db_upgrade return a similar error.

What am I missing here?
