I don't know what your io addresses or irq's are. Look them up in dmesg 
after a fresh boot. Eventually you will end up with something like this:

alias eth0 pcnet32
alias eth1 e100
options eth0 -o pcnet io=ioadder1 irq=irq1
options eth1 -o e100  io=ioadder2 irq=irq2
alias scsi_hostadapter ips
alias usb-controller usb-uhci
