I think mpage has problems with page 0. The interaction of mpage and gs definatelty have problems with page 0.
OpenOffice.org starts with page 0 too, which caused lots of problems. One printer here ends up dropping the job without printing anything.
I have yet to find a solution. I don't understand postscript well enough to debug effectively.
