It is very difficult to tell what are the conditions that trigger this
bug, but yes i've seen it too.
Maybe it's dependent on some weird hardware-dependent timing issues.

A factor that seems to enhance the probability is running "rpm -qa |
whatever" before doing a "rpm -Uvh".
