There are a couple more items that might be appropriate.
I'm not sure about Outlook 2000 but exporting 'Contacts' as
a .csv file from Outlook 98 is pretty flaky especially if
you use the "note" field.  I had to use a text editor to be
sure the various fields had the delimiters and quotes in the
right places.  This could be a long and painful cleanup job.
Also if you are importing to Evolution, it won't read in
.csv files directly.  You must run cvs2vcard.pl to convert
it to a vcard file and then import it.  See
